General practitioners can refer patients to The Healthy Brain and Memory Centre by completing our secure online referral form below.

Please provide the patient’s details, reason for referral and any relevant clinical information. You may also upload the signed referral, medication list, pathology results and supporting reports.

Once received, our team will review the referral and contact the patient directly to arrange an appointment.
