New Therapies for Alzheimer's and Mild Cognitive Impairment

 The Therapeutic Goods Administration (TGA) has approved donanemab (brand name Kisunla) for the treatment of people with Mild Cognitive Impairment (MCI) or early Alzheimer’s disease.


Patient eligibility

To be considered for treatment, patients must meet all of the following criteria:

  • Evidence of a “rapid forgetting” pattern of memory impairment, or an isolated cognitive syndrome caused by Alzheimer’s disease (for example, primary progressive aphasia or posterior cortical atrophy)
  • A diagnosis of mild cognitive impairment or mild dementia due to Alzheimer’s disease 
  • Minimal impairment in daily functioning 
  • A Mini-Mental State Examination (MMSE) score between 20 and 28 
  • Confirmed amyloid pathology in the brain, demonstrated by: 
    • cerebrospinal fluid testing via lumbar puncture, or 
    • amyloid brain imaging
    • Completion of genetic testing for ApoE4 
    • Patients with two copies of the ApoE4 gene are not suitable candidates for Kisunla

 

Funding and rebates

The medication and associated investigations are not subsidised by:

  • the Pharmaceutical Benefits Scheme (PBS) 
  • Medicare 
  • private health insurance 

All costs are therefore out-of-pocket.


Important information for patients and families

It is essential that patients and their families understand the following:

  • Donanemab slows the progression of Alzheimer’s disease but does not stop or reverse the condition 
  • Treatment should be combined with other evidence-based approaches, including physical exercise, cognitive stimulation, and broader brain-health strategies 
  • Common side effects include mild infusion-related reactions, which may be uncomfortable but are usually short-lived 
  • Less common but serious risks include brain swelling (ARIA-E) and brain bleeding (ARIA-H) 
    • These complications may require treatment and can result in temporary or permanent cognitive decline, disability, or, in rare cases, death 
    • Regular MRI scans are required to monitor for these risks 
    • Patients should seek urgent medical assessment if concerning symptoms arise between scheduled scans
       

Costs to consider

Before commencing treatment, patients should carefully consider the financial implications, which typically include:

  • ApoE genetic testing: approximately $200 
  • Donanemab (Kisunla): estimated at $4,700 per infusion 
    • Approximately $56,400 per year + GST 
  • Infusion administration costs: approximately $600 per session, typically over 18 months 
  • Amyloid PET scans: approximately $2,500 per scan, usually performed every six months to assess treatment response and determine whether therapy can be ceased 
  • Safety MRIs: typically 3–4 scans per year, at approximately $200 per MRI

None of the above investigations, medications, or infusion-related costs are currently subsidised by Medicare or private health insurance
